Recommendations After Death Of Parkway Worker

A coroner's inquest into the death of a 40-year-old man working on the Rt. Honourable Herb Gray Pkwy. makes six recommendations to prevent similar deaths in the future.

Guiseppe Serra died when he became trapped between two objects on a construction site back in June 2014.

Among the six recommendations, the inquest suggests providing written information in a language workers understand and implementing refresher courses for employees conducting high-risk activities.

The jury also recommends amending the Occupational Health and Safety Act to require employers to provide written procedures for the erection and dismantling of forms and have them accompany design drawings, and designating a worker to remove fasteners before rigging a hoisting device and letting other workers on the site know who that person is.

The inquest was held last week in Windsor and wrapped up on Friday.
