Lambton Public Health and Lambton Elderly Outreach have launched a new online referral network.
The system, offered through Caredove, provides secure and seamless access to services for aging residents in Sarnia-Lambton.
Supervisor of Health Promotion Anita Trusler said primary care providers can search for services and send referrals electronically to help reduce patient wait times.
"It could be anything from supports in the home -- cleaning your house, doing yard service -- to getting VON nursing or access to support to the Alzheimer Society," said Trusler. "There's a variety of services and we're just trying to grow this network currently so that people will be able to access those services more easily."
The Canadian Red Cross, Family Counselling Centre and March of Dimes are some of the other partners.
Trusler said the service will help residents age-in-place, which is a key priority in the Age-Friendly Sarnia Action Plan.
The program was funded by a grant through the Ontario Ministry of Health and Long-Term Care.
