Police said George Smit, 71, of Petrolia was driving a car that collided with a transport truck at Mandaumin Road and Courtright Line just after 1 p.m.
Another person was treated for minor injuries.
Anyone who witnessed the crash is asked to call Lambton OPP or Crime Stoppers.
The intersection was closed for several hours for an investigation.
