Mr. Ponte was last seen leaving Bluewater Health Saturday and police are concerned about his health and well-being.
He's described as a white male, 5 feet, 7 inches tall, 140 pounds with a thin build.
He was last seen wearing a blue sweater, blue baseball cap and grey sweatpants.
